Re: USD #500 Benefit Package

Welcome to USD #500!

As a new employee, you may be eligible to participate in USD #500's Benefit Program. Please confirm your eligibility with Human Resources before proceeding with enrollment.

Due to changes in the USD #500 benefits plan year, please be advised that all benefits selected during 2012 Spring Open Enrollment will be effective July 1, 2012 — December 31, 2012. All employees will be provided an additional open enrollment period during the Fall 2012 school year. Benefits selected during Fall 2012 open enrollment will be effective January 1, 2013.

Health and Welfare Benefits

USD #500 offers employees a wide array of benefits to choose from. It is important that you understand your choices and how to make the choices that you are best for your specific situation and individual needs. Through your initial enrollment, the annual enrollment, and when you have a qualifying change in status mid-year, you can make sure that the choices you make are continuing to meet your needs.

As an eligible employee, you are offered the following health and welfare benefits:

Enrollment Procedure

Enrollment for most benefits (medical, dental, vision, Section 125 and voluntary group term life insurance) is online. You have until the end of the month in which your coverage is effective to enroll. In some instances, you may have to wait until you receive your first paycheck before you can enroll.

Your enrollment is not complete until you have your confirmation number from the enrollment site.

As a new hire, this is your first opportunity to sign up for coverage and coverage is effective the first of the month following your date of hire. If you do not sign up when first eligible, you must wait for the annual enrollment period in May (for a July effective date). You may also sign up during the year if you have any life-changing events within your immediate family (marriage, divorce, birth, adoption, or loss/gain of coverage under another medical plan).

Enrollment for Income Protection and Critical Illness occurs only during the May Annual Enrollment.

Kansas Public Employees Retirement System (KPERS)

All USD#500 employees whose job calls for them to work at least 630 hours per year are required by Kansas statute to participate in this plan. They become eligible the 1st day of employment. (Full-time students, high school or college, are not required to participate.) This is a defined benefit plan. Other than purchasing service credit (discussed below) employees cannot add extra monies to this plan.
