HOW TO ENROLL — Active Employees
Enrolling on the web is fast and easy! Follow the steps below to make your plan selections:
- Complete the Enrollment Worksheet .pdf (2 pages) that was given to you with your enrollment toolkit. This will ensure that you have made all the necessary decisions prior to going online.
- Go to the enrollment website at www.cbizesc.com/kckps.
- Enter your Employee ID Number (4 or 5 digits, found on your paystub).
- Enter your PIN (if you have never been to the site or you have not changed your PIN, enter your entire birth date, i.e. 01011900).
- After you have entered your ID and PIN, click "Login".
- Once you have arrived at the main menu, it is time to make your selections.
- Follow the online instructions to enroll.
- Once you have completed your enrollment, a confirmation statement will appear on the page. Print this page for your records.
- If the information is accurate, click "Confirm". You will then receive a confirmation number. Write this number down or print the page and keep it for your records.
- If the information is incorrect, click "Change".
Once you receive your confirmation number, you have finished the enrollment process. Your enrollment is not complete until you have your confirmation number from the enrollment site.
IF YOUR DEADLINE TO ENROLL IS ABOUT TO EXPIRE AND YOU CANNOT GET INTO THE ENROLLMENT WEBSITE (www.cbizesc.com/kckps) CONTACT THE BENEFITS OFFICE BEFORE THE END OF THE MONTH IN WHICH YOUR COVERAGE IS TO BE EFFECTIVE!
Enrolling in Unum Short Term Disability and Critical Illness
You can only enroll in the Short Term Disability and the Critical Illness policies during the annual enrollment period held in the spring each year. You cannot enroll in these two plans online. To enroll in the Voluntary Short Term Disability and Critical Illness, please contact the Call Center number listed on the enrollment website or your enrollment worksheet — enrollment for these two plans is only available during the May enrollment.
