Voluntary Worksite Program — UNUM
For contract employees and non-contract employees working 30 hours or more per week, you are eligible to participate in the Voluntary Worksite Program through UNUM. You can elect to enroll in all of the plans or just one of the plans. You are responsible for the entire cost of the premium. You may enroll in the Life Insurance upon satisfying your intial new hire waiting period (1st of the month following 30 days of employment for most employees – please check with Human Resources to determine your benefits waiting period). You may only enroll in the Critical Illness and Short Term Disability policies during the annual enrollment period held in May.
Voluntary Group Term Life
- Convertible upon termination
- Portability option for those that qualify
- $100,000 Guarantee Issue for qualified employee
- $30,000 Guarantee Issue for qualified spouse
- $10,000 Guarantee Issue for qualified child(ren)

Voluntary Short Term Disability
- Secures up to 60% of your gross salary (not to exceed $5,000 monthly) in the event of non-work related disability
- Fully portable at same cost (you own the policy, even if you leave employment at USD#500)
Voluntary Critical Illness Protection
- Plan pays lump sum benefits from $5,000 to $50,000 upon diagnosis of a covered event
- Covered events may include Heart Attack, Stroke, Renal Failure, Permanent Paralysis, Major Organ Transplant, and Cancer
- Fully portable at same cost
- Family coverage available

To enroll in the Voluntary Short Term Disability and Critical Illness, please contact the Call Center number listed on the enrollment website or your enrollment worksheet – enrollment for these two plans is only available during the May enrollment.
